What you are actually paying for
- Visits and membership. An intake consult, follow-ups, and sometimes an annual physician or membership fee.
- Labs. Baseline bloodwork before you start, then repeat panels at the intervals your clinician sets. Some providers include these; others charge per draw. See the TRT bloodwork schedule for how often labs are typically needed.
- Medication. Testosterone (injection, cream, or oral), or an alternative like enclomiphene.
- Add-ons. Anastrozole to manage estrogen, hCG to support fertility, injection supplies, and shipping. Some bundle these; many bill them extra.
- Pharmacy and logistics. Shipping fees, at-home lab kit fees, and in-person draw fees.
A fair comparison adds up all five for twelve months. That is the "true first-year cost" in each of our provider reviews.
First-year totals, provider by provider
Sorted from lowest to highest published first-year figure. Read the last column: totals are only comparable when they include the same things.
| Provider | Headline price | First year | Per month | What the first-year figure covers |
|---|---|---|---|---|
| Hims (Rx+ enclomiphene) | From $49/mo + $49 membership | $637 to $1,176 | ~$53 to $98 | Medication plus the required membership (two labs a year included); the low figure assumes the membership is a one-time fee, the high figure that it bills monthly, which Hims does not specify. Compounded injectable testosterone is from $79/mo. |
| PeterMD | $79/mo (12-month plan) | $948 | $79 | Medication only; labs not clearly priced |

Three pricing models
- 1Flat, all-in monthly price. Henry Meds, JoinFridays, Wittmer, and Fountain bundle visits, medication, and (mostly) labs. These are the easiest to compare, and a published monthly price is close to the real cost. Check what "labs" means: a single baseline panel or all follow-ups.
- 2Membership plus medication. Male Excel and Hone Health charge a membership, and medication is separate (and, at Hone, unpublished until after your consult). The headline membership price is only part of the bill.
- 3Itemized. Defy Medical and Marek Health bill consults, lab panels, and medication separately. They can be good medicine, but the total depends on how many labs and visits your plan needs, so get a written estimate. Defy's own line items add up to roughly $1,200 to $2,950 in year one, against a company estimate of about $2,100.
Year two
Most programs cost less after the first year because the intake fee is gone and labs space out once your dose is stable. Defy says its average drops from under $175 a month to about $125 (about $1,500 a year). Nuform says the same about its lab costs. Flat monthly plans such as Henry Meds or Fountain should look much the same each month, so ask what changes in year two. If a provider is quoting only a year-one number, ask for the year-two price too.
Insurance, HSA/FSA, and in-person clinics
- Most telehealth TRT is cash-pay. Nuform accepts credit and debit only; Hims does not take insurance, and says testosterone treatment is not eligible for FSA or HSA reimbursement. Some providers accept HSA/FSA cards; confirm before you pay.
- In-person and hybrid clinics can use insurance. Low T Center, for example, accepts most major plans and offers a free initial consultation and test, with self-injection from $105/month. Coverage depends on your plan and diagnosis, and in-clinic pricing varies by location.
- Your own doctor. A primary care physician, endocrinologist, or urologist who prescribes generic testosterone may cost less if your insurance covers the visits, labs, and medication. That route also needs a confirmed diagnosis; see low T symptoms and diagnosis. Pharmacy discount cards can lower generic prices, but prices vary, so check them yourself.
- Labs on your own. Independent lab services can run the baseline panel without a program fee. Our sponsor Goodlabs offers discounted panels, and we may earn a referral commission if you sign up through our labeled link; Goodlabs does not provide TRT.
Work out your own number
- Ask for the first-year price in writing, including every lab draw you will need and how often.
- Add up the five parts: visits and membership, labs, medication, add-ons, and shipping.
- Divide by 12 to get the real monthly cost, and compare that, not the headline price.
- Ask for year two. Which fees fall away, and what do labs cost once you are stable?
- Check the exit. What is the refund policy, and can you cancel without penalty?

Sources and method
Every figure above comes from the pricing section of the matching review, which cites the provider's own published prices (or a labeled third-party estimate) and the date it was checked: September 21, 2026 for most, October 3, 2026 for Henry Meds and Maximus. Totals use the company's most common entry path and include only what the review says they include. Prices change, so treat this as a snapshot and confirm before you pay.
