Ageless Men's Health is one of the largest in-person testosterone clinic chains in the U.S., with more than 80 locations listed on its site. The model is simple and very hands-on: you walk in, a clinician takes your history, does a brief exam, and draws blood, and if you qualify you come back every week for an injection given in the clinic. Visits can be as short as a few minutes.
That weekly contact is the main strength. You're seen in person every week, the injection is given by staff rather than at home, and the protocol includes hCG to help preserve testicular size and natural production, plus a medication to manage estrogen. It also accepts most PPO insurance, which is rare in this directory. The trade-off is flexibility: injections are the only testosterone format, and you need to get to a clinic during weekday business hours.
Two cautions. First, pricing isn't published anywhere on its site, so you'll need to call your local clinic. Second, in 2015 the company paid $1.6 million to settle federal False Claims Act allegations that, from 2009 to 2013, it billed Medicare and Tricare for a medically unnecessary office visit each time a testosterone shot was given. It agreed to oversight by the HHS Office of Inspector General as part of the settlement. We haven't found later regulatory action, but it's worth knowing, especially if you'll use insurance.

Because you visit every week, your copay applies to each visit if your plan bills them separately. Ask your clinic and your insurer how visits and injections will be billed before you start, and get the self-pay rate in writing if you're paying cash.
